It evolves from Feebas when leveled up with its Beautiful condition high enough, or (from Generation V onward) when traded while holding a Prism Scale.
Biology

Milotic is an aquatic, serpentine Pokémon with a primarily cream-colored body. It has red eyes with long, pinkish antennae above them. Additionally, there are long, hair-like fins above its eyes, which begin thin and thicken towards the tips. These eyebrows extend to almost half of Milotic's body length and will be longer on the female than the male. On top of its relatively small head is a straight spike. Running down either side of its neck are three black dots, similar to gills. Its lower body is a pattern of blue and pink, diamond-shaped scales outlined with black. The tail consists of four large, blue fins with pink ovals in their centers. Each of the tail fins overlaps the next slightly, causing it to resemble a fan. Milotic's scales are said to change color depending on the viewing angle, which has so far only been depicted in Pokémon Battle Revolution.
Milotic is said to be the most beautiful of all Pokémon. In fact, it is said to have inspired many artists in their works. When there are fights going on, it has the power to calm angry feelings and end arguments by releasing a wave of energy. In addition, it can move by floating in the air as shown in the anime. It lives at the bottom of large lakes.

Major appearances
Milotic debuted in Win, Lose or Drew!, under the ownership of Robert. He used it for the Hoenn Grand Festival, where the Tender Pokémon helped him to fulfill his goal of becoming a Top Coordinator.
In The Great Eight Fate!, Juan's Milotic was used against Ash for their Gym battle in the following episode, being his strongest and last Pokémon. Milotic was able to defeat Swellow with a combination of Hydro Pump and Twister before losing to Pikachu's Thunder.
In Queen of the Serpentine, Lucy used a Milotic during her battle against Ash at the Battle Pike. It defeated Donphan before losing to Pikachu when he used Volt Tackle in one of its Twisters. It reappeared in a flashback in A Pyramiding Rage!.
In Malice in Wonderland!, a wild Mismagius created an illusion in which a Feebas owned by Johanna evolved into a Milotic while battling Dawn's Piplup during the world's greatest Coordinator playoff.
Wallace also owns a Milotic that appeared over the course of the Wallace Cup arc, starting in Our Cup Runneth Over!. It partook in the opening of the competition in Staging a Heroes' Welcome!, and when Dawn was awarded the Aqua Ribbon in Strategy with a Smile!. It also appeared in a fantasy in Goodbye, Junior Cup - Hello Adventure!.
Lusamine's Milotic appeared in Rescuing the Unwilling! and 10,000,000 Reasons to Fight!. When Nihilego possessed Lusamine, it was forced to attack Ash and his classmates before Nihilego was defeated.
In The Cuteness Quotient!, Jinny's Feebas evolved into a Milotic during a battle against Meowth. After blasting Team Rocket off, it helped its Trainer win the Friendship Festival.
In Sword and Shield: "From Here to Eternatus!", Oleana used a Milotic in her battle against Goh and Raboot. In Sword and Shield... The Legends Awaken!, it was defeated.

A Milotic appeared in the boss fantasy in A Lean Mean Team Rocket Machine!.
In Arceus and the Jewel of Life, Palmer used a Milotic during his battle against Cynthia and her Garchomp.
A Milotic appeared in the opening sequence of Zoroark: Master of Illusions.
A wild Milotic appeared in Alola to New Adventure!.
In Yo, Ho, Ho! Go, Popplio!, Lillie hooked a Milotic while fishing and struggled to reel it in. Ash jumped across to help, only for Milotic to jump out of the sea and hit him, sending him flying into the ocean.
A Milotic appeared on television in Night of a Thousand Poses!.
A Milotic appeared in a fantasy in A Test in Paradise!.
In The Cuteness Quotient!, a Milotic was an image on Professor Cerise's screen.

In Early-Evening Waves, Nessa encountered a Milotic during a photoshoot. Upon seeing it, Nessa decided to take a ride on its back, which helped boost her modeling career in the process. She also recognized it as the evolved form of a Feebas that she had met as a child.

A Milotic appeared in The Rival under the ownership of Palmer. It was used as his first Pokémon in his Battle Tower battle against Barry and his Heracross, defeating it with Dragon Pulse.

In It All Ends Now VII, Ruby's Feebas evolved into a Milotic after Ruby declared that she had inner beauty.
In Moving Past Milotic, Emerald encountered a wild Milotic in the 21st room of the Battle Pike, but he simply ran away from it due to the fact that it was wild. Lucy sends out her Milotic against Emerald's Starmie later in the chapter.
Cynthia has a Milotic that was first used against Cyrus in the Team Galactic HQ. It first appeared in Shunning Spiritomb.
A Milotic appeared in a flashback as one of the Pokémon trained by the International Police in Innocent Scientist.

A female Milotic can appear in the background of the Unova Pokémon League stage of the 3DS version. However, it does not interact with the stage like Reshiram or Zekrom would. Shaymin and Whimsicott might also appear.

Milotic also appears as a trophy exclusively in the 3DS version of the game.

Trivia
- In the official Prima Strategy Guide for Pokémon Ruby and Sapphire, Milotic's Ability was mistakenly listed as Torrent.
- Milotic seems to be a counterpart of Gyarados. Both are the serpentine evolved forms of weak fish Pokémon, and both appear in times of conflict, Milotic to calm, Gyarados to destroy. Both have base stat totals of 540, sharing the same HP, Defense, and Speed stats with Attack, Special Attack and Special Defense stats sharing the same values but in a different arrangement.
- Milotic may also be a version of Gyarados oriented to Pokémon Contests, since it was introduced in the third generation where Pokémon Contests debuted. This is fortified by Milotic's emphasis being on its beauty, while Gyarados' emphasis is on its power. In addition, Milotic is strongly associated with Pokémon Contests in the anime; all known male Top Coordinators have a Milotic as their signature Pokémon.
- Milotic is tied with Gyarados for the greatest increase in base stat total after evolving, at 340 points.
- In Pokémon HeartGold and SoulSilver, it is possible to evolve Feebas into Milotic without trading, even though no Poffin can be obtained and used in the game. This is done by visiting the Haircut brothers in Goldenrod City or by talking to Daisy in order to groom Feebas, which not only helps to raise its friendship, but also raises its Beauty. This is subtly stated by the brothers and Daisy in the games, but cannot be checked unless the Feebas is traded to Diamond, Pearl, or Platinum. However, this cannot raise Feebas's Beauty if its sheen is too high.
- In the core series games, Milotic is the only Pokémon with multiple ways of evolving from its pre-evolution in the same game that does not have different forms for each method.
Origin
Milotic seems to be based on a sea serpent and a mermaid due to its hair-like fins, pale upper body, and scaly lower body. It is also similar to an oarfish, which have very long bodies and red fins. Its gills are similar to those of lampreys and hagfishes. Its tail fin resembles a hand fan, and its colorful scales resemble stained glass. Its feminine appearance and history of being depicted in art indicate that it may be inspired by Venus, the Roman goddess of love and beauty. Its drastic transformation after evolving from Feebas may be inspired by the classic fairy tale The Ugly Duckling, where a small and feeble duckling is treated as an outcast by other animals until it eventually grows up to be a beautiful swan.
Name origin
Milotic may be a combination of Venus de Milo (a sculpture known for its beauty) and aquatic, melodic, milk, exotic, or lotic (a river ecosystem).
Milokaross may be a combination of Venus de Milo and kalloni (Greek for beauty) or Charis (Greek goddesses of grace and beauty).
