1st Runner-Up
The first Runner-Up of each edition of Miss Grand International is the second placer behind the candidate who is crowned as Miss Grand International (first placer). In some cases, she shall take over the title of Miss Grand International, if:
- The outgoing titleholder cannot fulfill her duties. This could happen and may result to resignation, giving up the title, or dethronement
- The titleholder is dethroned due to deeds that violate the organization's policies. This has only happened in 2015.
The second place finisher being designated as 1st runner-up has been awarded ten times (2013–2022).
This table lists the number of 1st Runner-Up titles by country. There are some special considerations:
- As Australia took over the Miss Grand International title in 2015 and later terminated her title in 2019 for participating in Miss Universe Australia 2019 pageant, it is unknown if the 1st Runner-Up position was taken by another candidate after the succession took place.
The current 1st runner-up is Engfa Waraha from Thailand, elected on 25 October 2022 in West Java, Indonesia.

Notes
- Ages at the time of Miss Grand International.
- Anea Garcia was dethroned because Garcia could not fulfill her duties during her reign as Miss Grand International.[3]
- Claire Elizabeth Parker, the 2015 winner, was originally placed 1st Runner-Up but was assumed as the winner after the original titleholder was dethroned. Later, Parker terminated her title in 2019 for participating in Miss Universe Australia 2019 pageant.[4]
- The 2020 edition was originally to be organized in Caracas, Venezuela but was transposed to Thailand twice after the COVID-19 pandemic.[10]
- Hiwot Mamo was dethroned because Mamo participated in Miss Universe 2014 pageant during her reign as 1st Runner-Up.[13]
